• Fall sports practices begin August 10. In order to participate the following forms must be submitted:
    Physical Form 
    (page 1 is to be completed by a doctor and is good for 2 years. Pages 2-5 MUST be updated yearly.)
    Athletic Code of Conduct
    Emergency Contact Form
    Concussion Protocol Concussion Protocol is informational, signature will be done on the pre-participation page

    Paperwork can be dropped off this week Wed. through Fri. from 8:00am-11:30am or 12:30pm-3:00pm in the Activities Office. Please enter through Auxiliary Gym entrance and go to the Activities Office. Paperwork must be received prior to practice on August 10th, once paperwork is turned in you will receive a tryout card.
